Key Responsibilities:

Manage a caseload of healthcare members with long-term care needs.
Conduct regular assessments (monthly and quarterly) through home visits (80% travel) and telephonic follow-ups (20% remote).
Collaborate with members, caregivers, and providers to develop and update personalized care plans.
Monitor the delivery of services and ensure that care is seamless and effective.
Coordinate referrals for necessary services, ensuring smooth transitions and no gaps in care.
Work closely with discharge planners, physicians, and other healthcare professionals to ensure coordinated care plans, including acute and long-term services.
Assist members with filing complaints, appeals, and resolving any concerns they may have regarding their care.
Stay on top of necessary documentation and assessments using electronic health records.
Required Qualifications:

Active RN license in the state of North Carolina, in good standing.
Bachelor's degree.
At least 2 years of care management experience.
Proficient with Microsoft Office and Microsoft Teams.
Valid North Carolina driver's license, as this position involves travel.
Preferred Qualifications:

2+ years of experience in care management or service coordination, preferably with a managed care organization.
Bilingual in English and Spanish is highly preferred.
