We are seeking a dedicated Patient Care Advocate to join our team in Stamford, where you'll work collaboratively with clinical staff to ensure a comprehensive and compassionate approach to patient care.
Our Patient Care Advocates: - Conduct thorough assessments of patient concerns and expectations, ensuring alignment with Mindful Care processes to enhance patient satisfaction and compliance.
- Maintain effective communication with patients, pharmacies, referral sources, hospitals, and therapists to meet patient needs and optimize retention.
- Support patients in obtaining medical records, completing necessary forms, managing prescription refills, and facilitating GeneSight testing.
- Secure prior authorizations from insurance companies for medications and services.
- Connect patients to the appropriate departments for their inquiries, anticipating the need for emergency interventions when necessary.
- Provide referral information to patients upon discharge from Mindful Care.
- Serve as a liaison between Mindful Care Patient Experience Representatives, Patient Operations, and psychiatric providers.
- Perform administrative duties and other tasks as assigned by management.
Qualifications: - Required: 3+ years of experience in administrative functions or case management.
- Preferred: Possession of BLS or CPR/First Aid certification.
- Preferred: Experience with Transcranial Magnetic Stimulation (TMS) administration.
- Required: Proficiency with Google Suite and Microsoft Office (Word, Excel, Outlook, Teams).
- Required: Associate degree required; bachelor's degree preferred (experience may substitute for a bachelor's degree).
- Required: Excellent verbal and written communication skills.
- Required: Reliable transportation and home internet access.
Position & Schedule: - Type: Regular Full-time, Non-exempt
- Hours: Monday-Thursday, 9AM-7PM (4 days a week, 10 hour shifts)
- Location Type: 100% in office
- Base Salary: $20.00 per hour
